Advanced Practice Clinical Nurse Educator (2531372318)
The Safehaven Project for Community Living (Safehaven) is looking for an experienced Registered Nurse to fill a key role of an Advance Practice Clinical Nurse Educator that works to align with our mission: To provide high quality, family centred and community-based residential and respite care for individuals with complex care needs.
- The APCNE will provide clinical supervision, consultation, and direct support (as required) within a community clinical setting. The APCNE is a member of Safehaven’s leadership team with core focus on education for staff, professional development, and collaboration with external partners in the community. Reporting to the Director, Programs and Professional Practice, the APCNE provides leadership and development of best practice guidelines, policies and procedures and clinical overview of client population.
§ Observes nursing and related practices to ensure compliance with standards and regulations of CNO, prepares reports of findings to guide program recommendations.
§ Demonstrates the ability to coach mentor and motivate staff to create an environment conducive to learning and team building.
§ Works closely with the interdisciplinary teams (internally and externally) to support the development of policies, procedures and best practice guidelines related to nursing practice and new programs;
§ Demonstrates ability to build partnerships and collaborative relationships with external community partners to support ongoing program growth and development.
§ Demonstrates the ability to prepare educational material and present to diverse audiences using various methods (in-person, virtually).
§ Demonstrate initiative and creativity in clinical practices and educational delivery
§ Identifies program needs, and projects the impact of changes on programs and services;
§ Ensures provision of family-centered nursing care to individuals with medical complexities, chronic conditions and life-sustaining technology in a home like-setting.
§ Assesses children's needs, collect and compile data, identify problems, develops, communicates, implements and evaluates care plans;
§ Works closely with interdisciplinary team members internally and externally in the clinical development and implementation of a transitional care program with community partners.
§ Provide evaluation and audits of client health care records offering recommendations for quality improvement.
§ Delegates appropriate tasks to registered nurses (RN), Registered Practical Nurses (RPN) and Residential Support Workers (RSW);
§ Follows all policies and procedures, established by the organization as they pertain to the operation of a residence, and the health, safety and welfare of clients in care, and as they pertain to employee conduct and performance.
Education and Experiences:
§ University Degree in Nursing (BScN); Masters an asset;
§ Registered Nurse with current certificate of competence from the College of Nurses of Ontario;
§ Minimum five (5) years of recent and relevant experience;
§ Previous Nursing experience in pediatrics; or working with chronic conditions, medical complexities and technologies is an asset.
§ Completion of a Basic Cardiac Life Support(B.C.L.S)program, Level II: Basic Rescuer certification;
§ Theoretical and practical experience in adult education including educational program planning, development and evaluation.
§ Demonstrates excellence in teaching, problem-solving and critical analysis
§ Demonstrate ability to communicate effectively with coworkers, interdisciplinary team members, clients and their families.
§ Strong communication skills and an ability to quickly adapt to evolving workplace environments
§ Understands and delivers to meet the goals and strategic objectives of the organization and the ability to quickly adapt to evolving workplace environment.
§ Strong computer skills, with excellent oral and written communication skills;
§ Successful applicants must provide a recent satisfactory vulnerable sector criminal reference check and medical clearance certificate
§ Travel is required for this position, the successful candidate must have access to reliable transportation;
§ Ability to perform delegated tasks independently or with minimal supervision.
