Position Title: Nurse Practitioner – Vanier Social Pediatrics Hub

Component/Team: Health Services

Status: 13 Month Contract (end date December 2022), 0.6 FTE Part-time (21 hours per week)

Salary Scale: $101,410 – $122,177 annualized salary plus benefits, both pro-rated to part-time status

Start Date: Immediately

Position Description

The Nurse Practitioner is a member of the Sandy Hill Community Health Centre Health Services team and works closely with team members to provide the highest possible quality of health care services to clients. The role of the Nurse Practitioner is to work with all clients to help them achieve their desired health goals. The focus will be to work with clients who access services at the Social Pediatric Hub located at the Vanier Community Service Centre.

The Vanier Social Pediatrics Hub’s Nurse Practitioner provides a child and youth centered, comprehensive and integrated, interdisciplinary and inter-sectoral, holistic health care approach. The Nurse Practitioner works with vulnerable children, youth and their families to support the optimal development of the child and youth, by respecting their rights and their interests. The Nurse Practitioner focus of intervention is based on the child’s needs, strengths as well as those of the family and the community.

Reporting Relationship

As an employee of the Sandy Hill Community Health Centre, the incumbent is responsible to the Director of Health Services and through him/her to the Executive Director.

As a Nurse Practitioner, the incumbent is responsible to meeting the licensing and regulatory requirements of the College of Nurses as the professional governing body.

Skills, Education and Experience:

Requirements For This Position Include

Education and Language

  • A Baccalaureate degree in nursing from a recognized university.
  • Certificate of competence from the College of Nurses of Ontario as a Registered Nurse in the Extended Class
  • Under the Centre’s designation to provide French Language Services:
  • French (oral expression): Advanced +
  • French (oral comprehension): Advanced +
  • French (reading comprehension): Advanced +
Fluency in English, both oral and written
Professional Experience
  • Three to five years’ experience in community and/or public health settings
  • Experience working with infants, children and youth 0-17 years
  • Experience working with high risk, homeless youth
  • Experience in mental health and addictions
  • Experience working with clients with diverse socio-economic and cultural backgrounds
  • Experience working within a harm reduction approach
Knowledge, Skills And Abilities
  • Knowledge of the impact of social/cultural/economic factors on the determinants of health and how they impact children/youth and their families.
  • Commitment to and knowledge of the principles of social pediatrics.
  • Knowledge, understanding, and sensitivity to the complexity of issues arising for vulnerable children/youth and families.
  • Excellent counseling skills
  • Excellent assessment skills in the physical, social and mental health and development of children and youth.
  • Strong crisis intervention skills, suicide intervention skills, organizational skills, communication skills and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment.
  • Ability to work collaboratively within and across multi-disciplinary teams and community partners.
  • Ability to work with clients presenting a wide range of health issues
  • Commitment to and knowledge of community-based health care
  • Demonstrated flexibility, sound judgment, initiative & creativity
  • Proficiency in the use of computers and various software applications
  • Respects and values the diversity of communities and individuals

Conditions Of Employment

Hours of work will be developed in collaboration with the successful candidate to ensure a healthy work-life balance. A schedule of work will be based on the Centre’s hours of operation and program requirements and may include some evenings.
